Making a referral to The Barn couldn’t be simpler.
Step 1:
To refer a young person to The Barn you simply need to send us a Common Assessment Framework (CAF) Pre-Assessment Checklist.
This basic form helps you provide information to support your referral. We use this information to both prioritise our work as as a base for our assessment; so please provide as much detail as possible.
You can download the checklist form by clicking the link opposite. If the young person already has a full CAF; this can be provided instead of the checklist.
It is also helpful to ask the young person where they would like to be seen and any convenient days or times. This will help us to plan convenient appointment.
Step 2:
Ring us on 01226 280532 and say you would like to make a referral. We will take some information from you, such as the name, date of birth and address of the young person.
You will be asked to email, post or fax us the CAF Pre-Assessment Checklist. We won’t be able to offer you an appointment until we receive the CAF paperwork as we rely on the information you provide to prioritise appointments, so please give as much information as possible.
Our assessment appointments are allocated twice a week. As soon as we have an appointment available we will contact you and/or the young person. We aim to see young people within 10 working days of receiving the CAF paperwork. In the meantime you may be offered support in working with the young person.
Step 3:
A Barn worker will meet with the young person and conduct a triage assessment to find out more about their substance use and what services they might want to access. We will inform you of whether the young person attended this appointment. Further information sharing is dependent on the young person’s consent, however, we will inform you when the young person is discharged from the service.
